  • Clinical context such as body area, condition, injury or surgery date, and funding pathway
  • Referral documents, reports or scans you choose to upload
  • Referrer details and clinical summaries
  • Provider service details, ABN and accreditation information

Some of this information may constitute health information under the Privacy Act 1988 (Cth) and applicable state health privacy legislation.

3. Why we collect information

We collect and use information to:

  • Assess referral suitability and identify appropriate participating rehabilitation providers
  • Contact patients, referrers and providers to coordinate referrals
  • Share relevant clinical and contact details with suitable providers for referral purposes
  • Maintain network quality and review provider applications

We only share information with participating rehabilitation providers where it is reasonably necessary for the purpose of facilitating a referral. We do not sell personal or health information. We do not share information for marketing purposes without your consent.

Providers who receive information through this platform are responsible for handling it in accordance with their own privacy and clinical governance obligations.

We store information in Australia using secure cloud infrastructure. We apply reasonable technical and organisational safeguards to protect against unauthorised access, loss or misuse. No online transmission or storage system can be guaranteed completely secure.

We retain information only for as long as reasonably necessary for the purposes described above, or as required by law. When information is no longer needed, we delete or de-identify it.
